INITIAL INTAKE FORMS: When you book your first appointment for chiropractic or massage therapy, you will be emailed an initial intake form to complete prior to your initial appointment. These forms include questions about your concerns, your health history, whether you have extended health care that we may be able to direct bill and office policy consents. It’s easy to forget these forms and put them aside, however the information is required by your health care practitioner. Intake forms MUST be completed prior to your visit. Any intake form that has not been completed prior to 24 hours of your appointment will receive an email reminder to do so. Additionally, you may receive a call from our friendly front desk staff the day of your appointment if it is still not completed.

DIRECT BILLING: We offer direct billing to most major insurance companies, including Canada Life, Green Shield Canada, Manulife, Medavie Blue Cross, Sun Life, TELUS Health, and many of those who allow third party billing to pay the practitioner/clinic. In order to set up direct billing please complete the information in your intake form.

Dr. Richelle Almeida-Knight

Dr. Richelle graduated with an Honours Bachelor Degree in Science from McMaster University. She then went on to receive her Doctorate of Chiropractic from New York Chiropractic College. Specializing in spinal health, musculoskeletal pain, and injury rehabilitation, Dr. Richelle combines hands-on techniques with a holistic approach to care. With a strong and dedicated commitment to improving quality of life, Dr. Richelle works closely with patients to develop personalized treatment plans that address both immediate concerns and long-term health goals. Passionate about empowering individuals to live pain-free, Dr. Richelle stays current with the latest advancements in chiropractic care to ensure the best possible outcomes. Cody graduated from Anderson College’s Massage Therapy Program in 2022. He has worked with clients experiencing sciatica, hyper kyphosis, hyper lordosis, TOS, spinal fusion, disc herniation and low back pain. Having a sports career and injury setbacks of his own, he understands the importance of rehabilitation, and also being proactive with your physical and mental well-being. Dr. Chris Eaglen B.Sc., D.C.

Dr. Chris was born and raised in Waterford, Michigan. He graduated from Central Michigan University with a Bachelor of Science degree in Health Fitness in the Prevention and Rehabilitation program. Dr. Chris received his Doctorate from New York Chiropractic College. Dr. Eaglen is certified and proficient in different techniques including: Activator Methods, Diversified technique, ConnectTx (instrument assisted soft tissue technique), RockTape Myofascial Taping, Cupping, Electro-therapy/Ultrasounds, and gait analysis.

Dr. Eaglen has treated many sports injuries from recreational player to elite athletes. He’s helped and provided care for all different sports from local hockey teams to golfers and tennis players. He’s also dealt with traumatic cases since a portion of his chiropractic career has been centered around motor vehicle accidents and coordinating patient care with neurologists and spinal orthopedic doctors. He has worked with patients from all ages suffering from many conditions such as disc herniation, radiculopathies, whiplash, sciatica, headaches, neck pain, low back pain, and other joint pains. Nigel is a a graduate of D’Arcy Lane. He has over a decade of experience specializing in deep tissue massage, fascial release, and chronic pain management. He has worked extensively with WSIB, motor vehicle accident (MVA), and sports injury cases. Nigel practices what he preaches, receiving regular massages and maintaining a healthy, active lifestyle. He enjoys working with clients and strives to take that extra step to personalize every session.
